THIS ADULT TESTIMONIAL RELEASE ("Agreement") is between and among U. S. Smokeless Tobacco Brands Inc. ("USSTB"), Ryan iDirect, a division of D.L. Ryan Companies, Ltd., ("Ryan iDirect") and the adult giving the testimonial ("Adult") Agreement as follows:

I, the Adult, hereby give my permission for USSTB to use my quote as a testimonial without limit in any and all media, and I acknowledge and accept no receipt of consideration for this permission.

I hereby forever release, discharge and agree to hold harmless USSTB, Ryan iDirect and each of their affiliates, officers, directors, employees, shareholders, successors and assigns, and all other persons using my likeness and quote in accordance with the terms hereof, from any claim or liability in connection with the creation or any use of these quotes, this project or the smokeless tobacco products involved including written copy and modified or edited versions of my statements that may be created in connection therewith.

I represent that I was of legal age to purchase and use tobacco products at the time I gave my quote and that I have the right to enter into this Agreement. I understand and agree that my quote, which may be modified or edited, may be used in smokeless tobacco advertising, and I consent to such use.
